HEALTH CARE    

Health care coverage is available the 1st of the month following completion of 520 straight time hours of employment for eligible employees and their dependents.  Employees can have only one employer paid source for health care coverage.  The health care options are as follows:

Traditional is the most widely accepted option.  Master Medical (MM) services are paid on a reimbursement basis.  The Master Medical (MM) deductible is $100 per person, $200 per family per calendar year.

HealthPlus of Michigan is an HMO.  Employees must select a primary care physician who will coordinate of medical services through referrals.
